League Stars - QLD Fair Play / NT Sports Voucher Redemption Form

This form is for participants in Queensland or Northern Territory who wish to use their State-issued Sports Voucher to cover the cost of registration to participate in a League Stars program.

IMPORTANT NOTES
* This form CANNOT be used for voucher redemption for Touch Football or Club Rugby League.  Please contact your relevant Club or Association directly for options of applying your QLD or NT Sports Voucher.  You can find contact detail on the Club Finder here.
* Once redeemed, vouchers CANNOT be refunded or transferred to another sporting organisation.

Queensland - FairPlay vouchers can only be used for League Star programs which run for 8 or more sessions. This is per the Queensland Government Terms and Conditions for FairPlay voucher applications.

Northern Territory - NT Sports vouchers can only be used for League Star programs which run for 6 or 8 (or more) sessions.
